Who can use Fox Oak?
Fox Oak subscriptions are available to anyone who meets these criteria:
- 18 years of age or older.
- Physically located in, Arizona.
- Desire for real results, and medical guidance.
What if I don’t already have a provider?
A Fox Oak team member will schedule you for a telehealth (two-way interactive video) consultation with one of our founders’ providers once you’ve completed and submitted a medical intake form. During your appointment, the provider will discuss your treatment options (if you qualify for therapy) and answer any questions you may have. In addition to this, the founder will educate you on how to take your medication and how to store it.
Please note: All Fox Oak telehealth appointments are conducted on a secure, HIPAA- compliant platform.
How do I pay for my medication?

We will only bill you once a month for services rendered, including any medications that are shipped. You can conveniently log into your Fox Oak account to view and download copies of your billing invoices.
Do I have to sign a long-term contract with Fox Oak?
No. Our flexible programs give you a variety of contract and payment options, including yearly and month-to-month memberships. Once enrolled in a program, our knowledgeable customer support representatives are available Monday through Saturday, from 8 am to 5 pm (MST), to answer your questions.
What if I need to cancel my Fox Oak subscription?
While all our medications are clinically proven to be safe and effective, results may vary depending on the individual. Furthermore, Fox Oak understands that health issues or life changes may require you to cancel your membership.
You can cancel your Fox Oak subscription at any time through your customer account or by contacting our customer support team. Please note that canceling before your contract term has ended will require payment for any remaining contract balance.
Patients unable to continue due to health-related issues, as determined by a healthcare provider, may be eligible for an exemption. Once approved by us, the patient can cancel their membership without incurring further charges. Contact our customer support team for additional information about our cancellation policy.
